A client recently asked me to write an email informed consent form, which I have always recommended when using email to communicate protected health information (“PHI”) with patients or clients. But rather than merely taking the informed consent form from my HIPAA Documents Resource Center CD, 5th edition, and putting in the client’s name where it reads [name of entity], I did some additional research and made some changes to the email informed consent form to reflect the current status of email. Then, I reverse engineered it and took the client’s name out and replaced it with “[name of entity]” so that my Premium Members would have the latest version.
